Its a 2nd cvar that can be used for another ad network. No suitable one has been found as of yet.

Opens ads in background motd iframe every second forever on every client except admins to generate impressions. Ads are not seen by clients, but may be heard. Bandwidth will be used by clients to view the ads, many are flash.

IMHO not a very nice thing to do to clients, probably will lag them, or degrade their experience.
May force clients to disable html motds, which will hurt other server ops that use them for rules, welcome messages, and other modifications.
Not a nice thing to do to ad providers, as ads are not being seen.
Good way to get valve to disable background motds and ruin things for us all, as anyone could abuse clients by sending them inappropriate or unwanted data... I mention this because music mods and radio use this.

Anyways, looks likethe ops urls will be opened if you run this. You should at the very least not include your own urls as the default convar setting(s)....

With the 1 second timer ads basically don't have time to stream audio or video file so sounds are not an issue. When there is no ads available clients are redirected to google as a default page.

The ad provider is fine with the way the ads are being used. I had contacted them asking this very question before I started using them myself.

This was my email to them:
"I run a game server and have the ability to setup automation in such a way as to send my visitors anywhere I want multiple times while they are connected. Would your system accept multiple hits from the same users over an over again or would you not suggest doing this?"

This was the reply I got back:
"Several of our advertisers are willing to pay for more than one visitor per IP per day (within reason), and those that aren't have traffic filtered out as soon as they reach their limit. When our system no longer has ads for that IP, the visitor can be passed back to you..

From what you've sent so far, it appears that although the RPC isn't especially high, we have coverage for all of it, and none is being screened."

Did you inform them that all of the advertisements would be opened in the background, and none would be seen or heard?
