Cant see anything wrong with your config, perhaps the sm_cvar lines would be better placed in your server.cfg and wont effect other game modes.

Add this line at the end of your config
sm plugins...

The steamp app id is in the "Install or Update" shortcut and is 222860 (this is not the reason for your error)

You need to create a shortcut to srcds.exe

view the properties and add this...

While it has crashed on this chapter and in the safe room on my servers, I don't consider it a particular issue with this chapter as it does not usually fail. Its difficult to say but maybe 1 in 30+...
