The binary is the smx you drop into the plugins folder. I do not have "Thirdpersonshoulder Shotgun Sound Fix" but do have the "ThirdPersonShoulder_Detect" plugin and with or without it, I can't get...

If you didn't modify the source, where did you get it from? I'd like a link to it because the source in the top post doesn't line up with your errors.
