Okay guys, did a ton of testing. found out which plugin!

Its: basecomm.smx

Finally.

edit: there's more then one causing the issue

On CSS, MetaMod 1.8.3V works by itself but I had to turn off SourceMod 1.3.4 (just stock plugins and thats it) because the server would crash everytime someone joined.
