What have you set mp_roundtime to?
Try
mp_roundtime 1.5 or 2.0 or 2.5 .......

I think that is your problem.

I have it at 2.06 for mp_roundtime is that not acceptible?

No
Can't you have to 2.0 instead ?
Because then PTB will work.

Is there anything with my settings currently that can be altered to make it work better on a 21 slot server?

Yes
amx_ptb maxdiff 0 // maximum team size difference
Should be changed to
amx_ptb maxdiff 2
Because you have
amx_ptb maxsize 11
If you change the teams can be
11 vs 9

The way you have it now it can only be 10 vs 10

I assume you have the 21:th slot hidden or reserved for admins.

btw did it help to change mp_roundtime?

Yes, I changed the roundtime. Actually, it has 22 slots but 1 is hidden for reserved slot. Ok, max diff now changed.

I've set ptb_saychat to "1" but there are no chat messages...

The HLSW thingy don't work for me, too.
